As parents or guardians of the passengers transported by Capital City Limousine, we acknowledge by signing this pledge that:

• ·        Underage drinking in our limousines is illegal and will not be permitted by Capital City Limousine Service or its Chauffeurs.  We agree by signing this pledge that the services of Capital City Limousine for this event will be used for a Strictly Alcohol & Drug Free Evening.

• ·        We agree that the solid divider will stay open and moon roof will be locked at all times during the service.
• ·        Capital City Limousine has the right to cancel the contract with no refund if any of these rules are broken. 
•  A minimum charge of $250 will be added to your total for any damages to the vehicle caused by intent or neglect.  Additional charges may incur depending on the severity of the damage(s).    

       Parent(s) acknowledge and have agreed to and confirmed all of the arrangements for the event.   The parent(s) ____________ (do/do not) want the chauffeur to follow the original plans and drop off time as outlined in the Request for Service.  If the parent(s) Do Not want the chauffeur to follow the original plans and additional stops or driving around is requested by the student an additional hourly fee of $ ________ will be added on and due at the end of the night, for the unscheduled time the Chauffeur does with the parent(s) permission.      

      If any terms of this pledge are broken by the passengers in the vehicle, the Chauffeur will immediately contact the parent(s) listed and all passengers will be immediately returned to the location where the service originated.  The service and remaining time will be forfeited and no refunds will be given.
